| Foundation Park Alzheimer’s Care Center was established in 1989 as the first free standing center in Northwest Ohio specializing in the treatment of Alzheimer’s disease and related dementias. The center is fully licensed by the State of Ohio Department of Health to serve 125 residents and it is approved by the Medicare and Medicaid programs. Foundation Park Alzheimer’s Care Center is owned and operated by Nursing Care Management, a Cincinnati Ohio based health care company. |
